(005) A Systematic Review of Preventative BRCA Surgeries and Sexual Function Outcomes

Abstract Introduction BRCA 1 and 2 genes increase a patient’s risk for developing cancer of breast ovarian origin. There are two methods preventative care offered to BRCA-positive patients: prophylactic surgery or high screening accompanied by endocrine therapy. Preventative surgeries patients – bilateral salpingo-oophorectomy (BSO) mastectomy - have been linked decreased sexual function dyspareunia. A decision whether not undergo preventive as opposed (with without hormone therapy) is heavily reliant on the advice received from their health providers. The association risk-reducing BSO with quality life, including postoperative dyspareunia, should be taken into account when conducting shared decision-making patients. Objective objective this systematic review assemble analyze findings related patient reported after these surgeries, see if how either affects dysfunction baseline, compare effects surgical procedures can mitigated replacement Methods literature using Pubmed, Embase MEDLINE databases inception through January 25, 2022 was conducted. Included in search were any form observational study, clinical trial article type. Conference abstracts also included. To included, studies had meet an priori list Medical Subject Headings (MeSH) terms “BRCA” AND “sexual dysfunction” OR “dyspareunia”. Results yielded 13 results, which seven sufficient data review. Participant numbers ranged 15 804. Sexual dyspareunia measured via both validated investigator-generated surveys, three used Female Function Index (2 BSO, breast) Activity Questionnaire (both BSO). All studies, no matter survey metric, found there significant post surgery. but one that therapy show improvement function, all op could reach baseline level Menopausal status moderator. No screenings effect future look group possible study control. Conclusions our knowledge first incorporate reducing mastectomies among help making positive clinicians choosing between screening.
